Exposure and Response Prevention (ERP) is the gold-standard psychological treatment for Obsessive-Compulsive Disorder (OCD).

ERP is a specialized form of Cognitive Behavioural Therapy (CBT) designed to help individuals break the cycle of obsessions and compulsions.
In OCD, a person experiences intrusive thoughts, images, sensations, or urges (obsessions) that trigger anxiety, distress, guilt, uncertainty, or discomfort. In response, the person engages in compulsions — behaviours, mental rituals, avoidance, reassurance-seeking, checking, analyzing, or other strategies intended to reduce distress or gain certainty.
ERP works by helping individuals gradually and intentionally face feared thoughts, situations, sensations, or triggers (the “exposure” component) while resisting compulsive behaviours and rituals (the “response prevention” component).
What the Brain Learns from ERP
Through repeated practice, the brain learns several important things:
- Anxiety and discomfort can decrease naturally without compulsions
- Intrusive thoughts are not dangerous and do not require a response
- Uncertainty can be tolerated
- Compulsions actually maintain OCD over time
- Fear predictions are often exaggerated or untrue
ERP is not about “getting rid” of thoughts or forcing someone to feel calm. Instead, it focuses on changing the person’s relationship to uncertainty, fear, and intrusive experiences.

- Touching a feared object without washing
- Leaving the house without checking repeatedly
- Allowing intrusive thoughts to exist without analyzing or neutralizing them
- Purposely triggering uncertainty while resisting reassurance-seeking
- Staying present with uncomfortable bodily sensations without monitoring or avoidance
ERP is typically done gradually and collaboratively with a trained therapist. Treatment is tailored to the individual’s OCD themes, symptom severity, and goals. Over time, ERP can significantly reduce OCD symptoms and help individuals regain freedom in their daily lives.
